As part of your application, please send your CV and the details of your Affiliate Package to Georgia and Wendy.

Affiliate Package details should include:

Whether you intend to sell individual or group packages (or both).
The scope of your coaching package e.g. whether you will include any other services like Physio or Pilates.
How regularly you will coach your clients and the mechanism you will use (e.g. face to face, email, zoom call etc).
How you will market your package.
The cost of your package.
Your pre-screening form - it is very important for us to ensure your pre-screening form is sufficient to complete appropriate risk stratification.

I confirm Wendy and Georgia received a copy of my case study client's results as part of the Menopause Practitioner Course.

I confirm I have the appropriate indemnity insurance arrangements to work with clients.

I understand that if I have not completed the MyMT™ Transform Me, Circuit Breaker or Menopause Weight Loss Coach courses, I will be required to pay NZ$99 as a one-off joining fee. This joining fee enables us to provide you with access to MyMT™ Transform Me and Circuit Breaker so you can coach your clients through them.
